Include your name (as it is listed on your State License Card), email address, telephone number, verification of hours (copies of certificates/courses taken for credits). ALL COMPLETION CERTIFICATES WILL BE EMAILED DIRECTLY TO THE EMAIL YOU PROVIDED IN THE REQUEST. NO CERTIFICATES WILL BE PHYSICALLY MAILED.
